Case Manager / Social Worker

Full job description

Job Summary

Duties

Assist the social services department in planning, developing, organizing, implementing, evaluating, and directing of social services programs in the facility.
Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
Participate in discharge planning, development and implementation of care plans and resident assessments.
Encourage residents/responsible parties to participate in the development of their care plan, and invite them to care plan meetings accordingly.
Will accurately and completely document social service actions and interactions in each resident’s medical record.
Assist with facility policy development and annual review in order to positively impact the quality of care delivered to residents.
And all other tasks assigned.

Qualifications

A degree in Social Work, Psychology, or any human service related field is preferred; relevant experience may be considered in lieu of formal education.
A minimum of a high school diploma or equivalent.
Self motivation and willing to learn independently when given resources.
A solid understanding of technology, or willingness to learn.
Ability to make independent decisions when circumstances warrant such action.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel and the general public.
Strong Leadership Skills
Organized and efficient
Maintain composure in stressful scenarios.
A commitment to continuous professional development and staying current with industry trends.

Part Time and Full Time CNAs - All Shifts

Certified Nursing Assistant (CNAs) – Afternoon & Midnight Shifts: $20.07-$22.60 hr.

Job Summary
Join our compassionate care team! CNAs at HCMCF provide essential hands-on care and support to our residents, helping with daily activities, monitoring health, and ensuring comfort during their stay. We're currently hiring for afternoon and midnight shifts—perfect for those who value a quieter environment and meaningful resident connections!

Key Responsibilities May Include and Not Be Limited To:

Assist residents with activities of daily living (bathing, dressing, grooming, toileting, feeding)
Monitor and record vital signs and report changes to nursing staff
Respond to call lights and resident needs promptly and compassionately
Help with mobility, transfers, and positioning
Maintain a clean, safe, and supportive environment
Support residents’ emotional and social well-being

Qualifications

Current Michigan CNA certification required
Compassionate, dependable, and team-focused
Ability to work afternoon (2nd shift) or midnight (3rd shift)
Must pass background check and physical screening

Casual LPNs and RNs

Casual LPNs ($30.40-$32.65hr) and RNs ($36.95-$39.20hr)

Key Responsibilities May Include and Are Not Limited To:

Deliver compassionate, high-quality care to residents in accordance with care plans
Administer medications and treatments as prescribed
Monitor residents’ health status and communicate changes to the care team
Collaborate with CNAs, physicians, and other healthcare staff
Ensure documentation is accurate and up to date
Maintain a safe and respectful environment for residents and staff

Qualifications:

Valid Michigan RN or LPN license required
Long-term care or skilled nursing experience preferred
Strong communication and critical-thinking skills
Ability to work flexible, on-call hours including weekends or holidays as needed
